(Dec. 7, 2011) -- LBReport.com is told by the state agency involved that the elevator in the CSULB Foundation building that was the site of a fatal injury this morning (Dec. 7) was inspected (along with three others in the building) on April 5, 2011. Elevators are required to undergo inspections annually, said Erica Monterroza, spokesperson for the CA Dept. of Industrial Relations and its inspection paperwork is up to date and in order. (Its next regularly scheduled inspection wasn't scheduled until April 2012).
Investigators from the CA Department of Industrial Relations, as well as Cal OSHA [since a CSULB employee was victim] will be conducting investigations into what happened earlier today. LBReport.com began receiving emails nearly immediately after CSULB communicated sent students the following email in the 10 a.m. hour: "There has been a fatality on the CSU Long Beach campus today as the result of an accident. The matter is currently under investigation. The name of the deceased is not being released at this time pending notification of the next of kin." . LBReport.com confirmed that it was an elevator accident in the Foundation building and moved the story on our front page within minutes.
